All participation forms below need to be completed in order to be eligible to participate in the athletic program.

LTISD Sports Participation Physical Evaluation 25-26 (Must be dated April 1 2025 or later) - Upload the completed form into the Rank One Portal

LTMS Drug Testing Consent Form 25-26  (Mandatory for students in grades 7-12 participating in school-sponsored competitive extracurricular activities) - This form is not available at this time.
