LTISD Participation Forms

All participation forms below need to be completed in order to be eligible to participate in the athletic program.

Physical & Medical History Forms (Must Be Filled Out To Participate) - Dated Feb.2026 or later

• Once completed upload it into the Rank One Portal.  You will need to create an account if you have not already done so.

RankOneSport - Online Forms (Must Be Filled Out To Participate)

• Drug Testing Consent can NOT be completed in Rank One.   Coaches will manually check these off in August 2026.

LTISD Drug Testing Consent Form 26-27 - This will be posted and emailed once finalized.  (Mandatory for students in grades 7-12 participating in school-sponsored competitive extracurricular activities).  Coaches will manually check these off in August 2026.
