Athletic Participation Forms 2024-2025

All participation forms below need to be completed in order to be eligible to participate in the athletic program.

LTISD Sports Participation Physical Evaluation 24-25 (Must be dated April 1 2024 or later)

Online Forms 2024-2025  RANK ONE SPORTS -  (Forms Completed Online) 

• Online Forms: ECC, Medical History, Emergency Travel, Strength and Condition, UIL Signature Page

LTMS Drug Testing Consent Form 24-25 (Mandatory for students in grades 7-12 participating in school-sponsored competitive extracurricular activities) - Posted 7-11-24

Hard Copies can be scanned and emailed to coaches in PDF Format.  Label the PDF file with the athlete's name.
